Before you begin, please review the form and verify that you have all of the requested information available.  In particular, you will need to know your medical insurance carrier and policy number.  Reading the form carefully and providing complete responses will help us provide you with the best OA experience.

TRIP PREFERENCE

If you have not already done so, please take a moment to review the information on the trips page (new window) to help you decide which adventures you would enjoy the most.  While we make every effort to assign participants to one of their most highly preferred adventures, it is not possible to guarantee a place on a particular trip.  Camping reservations, hiking permits, and service contracts are arranged as much as a year in advance, so it is generally not possible to mold our trip offerings to the preferences of the incoming class.  In the past, we have been able to assign all participants who sign up by the initial deadline of May 15th to one of their top three trip choices.

Athletes may only participate in one of the asterisked ( * ) adventures below.

Please rank your choices in order of preference (1-15, 1 being the most preferred).  A rating of 'N/A' indicates that you would rather not participate in the program than be assigned to this trip.  If you prefer two trips equally, you may assign them the same number and continue ranking subsequent preferences with the next consecutive number (e.g. 1,1,1,2,3,3,4,...).  Please rank all trips:

Your answers to the following questions will help us assign you to a group that will enjoy roughly the same activities and level of exertion as you (most trip categories are divided into several groups).  Some trips require participants to be able to swim, ride a bike, and/or be in good cardiovascular condition.  Please see Choosing a Trip (new window) for details on the activities and experience required for each trip.  An answer of 'N/A' indicates that you can engage in the noted activity only with great difficulty or not at all.

Please rate your swimming ability on a scale of 1 to 5, with 1 indicating little or no swimming experience and 5 indicating lifeguard or competition swimming experience.

   
Please rate your hiking ability on a scale of 1 to 5, with 1 indicating little or no experience hiking and 5 indicating your ability to hike on varied terrain and for long distances.
   
Please rate your skill riding a bike on a scale of 1 to 5, with 1 indicating little or no experience on a bike and 5 indicating your ability to ride on varied terrain and for long distances.
   
Please indicate your level of cardiovascular fitness on a scale of 1 to 5, with 1 indicating that you rarely or never engage in cardiovascular conditioning and 5 indicating that you are in top condition and can easily jog for several miles.

Several adventures require you to bring along a sleeping bag, sleeping pad, and/or frame pack.  If you do not own your own gear, you can rent gear from OA.  If you do own your own gear, we ask that you use it as there is only a limited stock of equipment locally available for rent.
